Initial experience of partial cystectomy with transurethral laparoscopic technique for bladder carcinoma

چکیده
Background: To establish a new technique for en bloc resection of bladder carcinoma using a transurethral laparoscopic technique. Methods: Nine patients with primary bladder tumors underwent Partial Cystectomy with Transurethral Laparoscopic Technique (PCTLT). A 22 Fr Peel-Away sheath was inserted into the urethra for endoscopy, fluid discharge, and tumor removal. A 20.8 Fr nephroscope was used for observation. The operating channel of the nephroscope was utilized for appliance placement and perfusion. PKSTM Plasma-CutTM was used for tumor excision and haemostasis. Ultra-thin scissors were used for tumor separation and cutting. A laparoscope-like technique was utilized to perform en bloc resection of the tumor and the entire muscle layer at the base, as well as the surrounding tissues within 0.5 cm of the tumors. Intraoperative hemorrhage, operation time, obturator nerve reflex and postoperative complications were observed, and pathological stage and postoperative follow-up were performed. Results: This technique was successful in all patients. No patients experienced bladder perforation or required open surgery. Perioperative blood loss was minimal and obturator nerve reflex was not obvious. Pathological stage of the tumors was Ta or T1. Patients were followed for 4 to 8 months postoperatively with no recurrence at the resection site and other site. Conclusions: Preliminary clinical results have demonstrated that PCTLT is a safe and effective treatment of Non-Muscle Invasive Bladder Cancer (NMIBC), which contributes to the pathological staging and clinical treatment of the bladder carcinoma, whereas middleand long-term clinical efficacy remains to be elucidated.

Laparoscopic partial cystectomy in bladder cancer -- initial experience.
PROPOSAL The authors present their initial experience with a selected group of patients who underwent laparoscopic partial cystectomy for treating bladder cancer. MATERIALS AND METHODS In the period from June 1997 to April 2000, 6 patients, aged between 38 and 76 years, having transitional cell carcinoma of the bladder, were identified as candidates to partial cystectomy. The procedure employ...
متن کاملInflammatory Myofibroblastic Tumor of the Urinary Bladder Managed by Laparoscopic Partial Cystectomy
Inflammatory myofibroblastic tumor of the urinary bladder is a rare mesenchymal tumor with uncertain malignant potential. It often mimics soft tissue sarcomas both clinically and radiologically. Surgical resection in the form of partial cystectomy or transurethral resection remains the mainstay of treatment. Herein we report the case of an inflammatory myofibroblastic tumor in a young girl, whi...
متن کاملLaparoscopic radical and partial cystectomy
Radical cystectomy remains the standard treatment for muscle invasive organ confined bladder carcinoma. Laparoscopic radical cystoprostatectomy (LRC) is an advanced laparoscopic procedure that places significant demands on the patient and the surgeon alike. It is a prolonged procedure which includes several technical steps and requires highly developed laparoscopic skills including intra-corpor...
متن کاملNecrotic mass after transurethral resection of a bladder tumor: novel management with robotic partial cystectomy
A 76-year-old female with a history of high-grade transitional cell carcinoma (TCC) of the bladder presented with persistent nocturia and urge incontinence and was diagnosed with a necrotic bladder lesion. Cystoscopy revealed a 4 cm area of necrosis, ulceration, calcification, and fat. Transurethral biopsy confirmed the lesion to be benign, and two attempts to re-epithelialize the area of necro...
متن کاملIs TURBT able to cure high risk recurrent superficial or muscle invasive bladder cancer: factors resulting in pT0 radical cystectomy specimens.
PURPOSE In 76% of radical cystectomy patients there is discrepancy between the initial stage at transurethral resection and the final pathological stage of the cystectomy specimen. More specifically in contemporary series the absence of tumor at radical cystectomy specimens (stage pT0) is estimated at 5-25%. Our aim was to determine which factors contributed to the absence of tumor in our serie...
